POSITION SUMMARY

To provide patient care coordination for a smooth transition of Medicare patients between the acute hospital setting and Deaconess HomeCare. To obtain referrals and enact the 7-step process of the STEP Program as defined, in an effort to coordinate appropriate care, conduct appropriate assessment and education, and decrease/prevent Deaconess HomeCare hospital admission rates along with decreasing/preventing acute hospital readmission rates. To oversee an overall objective of increasing the quality of treatment delivered while adding to the efficiency of healthcare resource management with successful functional outcomes for patients.

Job Requirements

Education

Graduate of school of nursing, Bachelor of Science in Nursing preferred

Experience

minimum of three years nursing experience

Certificates & Licenses

Current licensure or permit by the state Board of Nursing, drivers license, automobile liability insurance and CPR certification.

Other Requirements

Knowledge of nursing theory, practice, and principles. Ability to effectively implement and document nursing care and evaluate outcomes of care. Effective written and verbal communication and interaction skills.
