LPN Home Health Scheduler & Licensed Practical Nurse at Hixson

Job Description

NHC HomeCare is seeking a well-organized, energetic Licensed Practical Nurse /LPN to work as a HomeCare referral intake/ scheduler & a Licensed Practical Nurse/LPN to work in our Chattanooga, TN office. The HomeCare referral intake/scheduler will work full-time in an office setting, working with our other clerical and clinical staff in order to serve our patients. 

The  Licensed Practical Nurse will be making visits to our homecare patients. This person is responsible for providing home care nursing visits within the plan of care. Home care is a flexible somewhat autonomous position for the LPN-reporting to the RN, but providing home care mostly independently on a daily basis. The LPN/Licensed Practical Nurse would work Full-time PPV.

Job Requirements

The LPN Scheduler position requires LPN license in the state of Tennessee/ a detail oriented person with data entry/computer experience and the ability to communicate well with referral agents, our patients (by phone) and clinical staff. HomeCare experience is a plus. Data entry with accuracy is necessary with extended time at a computer and on the phone.

The Home Health LPN position requires current licensure as Licensed Practical Nurse in the state of Tennessee with one years experience preferred in acute care and /or home care. Have ability to provide nursing interventions, drive with dependable transportation, and lift 70-80 lbs on a diaily basis is necessary.
