LPN Case Manager at Oak Ridge

Emory Valley Center 715 Emory Valley Rd Oak Ridge, TN 37830 "Maximizing Human Abilities" Emory Valley Center is a non-profit agency in Oak Ridge that serves individuals with intellectual and developmental disabilities. We are currently seeking positive and professional LPN Case Manager to help us fulfill our mission of enhancing the lives of children, adults, and families, in an environment that promotes independence, dignity, and respect. The LPN Case Manager will need experience in the medical field, preferably as an LPN. Case Managers assist with Dr. appointments, Circle of Support meetings, medication reviews and other requirements in supporting the individuals. The Case Manager will coordinate the effort of EVC staff and outside professionals for the benefit of the individuals supported, will develop plans to support individuals, and will maintain comprehensive personal records and files related to the provision of Person Centered services and supports for an assigned case load. This individual also serves as an advocate and liaison for and on their behalf. Must be at least 18 with a high school diploma (or equivalent), an acceptable background check, driving record, and drug screen, TN Licensed Practical Nurse with Case Management experience preferred.
