Homecoming Caller at Brentwood

Job Description

Cogent Healthcare is seeking to add a full time Homecoming Caller to our corporate office team in Brentwood, TN.  The Homecoming Callers provide follow up calls to Cogent patient’s welcoming them home, identifying any issues, assure understanding of discharge orders, follow up appointment, assist in the prevention of rehospitalization,  patient surveys and maintain continuity of care.

Essential Functions

• Initiates outbound calls to Cogent’s eligible discharged patients to welcome them home and identify any issues or concerns to be referred back to the Clinical Care Coordinators for that market.
• Verify follow up appointment, medications, any home health needs, durable medical equipment needs, dietary or physical restrictions. Patient satisfaction survey.
• Maintain hourly call rate of 26 calls per hour.

Job Requirements

Education, Training and Experience

• High School Diploma or equivalent
• Minimum 1 year Customer Service Experience in a position that required outbound calling 
• Proficient use of computer skills/equipment
• Understanding of medical terminology
• Professional demeanor
• Ability to articulate in both written and verbal communications- required 
• Team player

Shift:

• Monday – Friday (40 hours per week)
